What you will learn
- Write time-bounded observations with identifiable locations and evidence bases.
- Reconcile conflicting information without erasing the disagreement.
- Document a later clarification while preserving the original observation's limits.
02.01
Describe the activity at the level you actually observed
Civil construction records become misleading when a narrow observation is written as a broad conclusion. A visit to one inlet does not establish the condition of an entire drainage run. A visible label does not establish every property of a component. Begin with the project, asset or station reference, observation period, activity, and the part of the activity you could observe. State a limitation when it affects interpretation.
Iris can describe six components at staging area B during the 09:00–09:25 visit. That does not establish the total delivered before arrival or subsequently installed. Use the project's location convention; pair a useful field nickname with the controlled asset or area reference.
02.02
Attach each quantity to its basis
Numbers require context. Record whether a quantity comes from an observed count, a supplier ticket, a measurement record, a contractor report, or a calculation. Preserve units and the portion of work represented. A ticket may describe a shipment; a note may describe visible stock at a later time. The numbers can differ without either source necessarily being wrong.
For assigned measurements, identify the feature, reference points, units, applicable method or instrument reference, time, and source worksheet. Separate recorded values from comparison with project criteria. This course supplies no tolerances or measurement method. If the criterion or review responsibility is missing, record that gap instead of declaring compliance.
In the case, write 'DT-448 states eight components; six components observed in staging area B at 09:05.' Do not convert this into 'two missing' or 'eight installed.' Both would introduce an unsupported interpretation. The next useful question concerns the difference between the shipment record and the later observed stock.
02.03
Separate reported information from your finding
A named communication may explain where to look next, but it should retain its source. Record who provided the information, when, the relevant statement, and what remains unverified. Avoid vague phrases such as 'it was confirmed' when the reader cannot tell who confirmed what or on which basis. Quote only when exact wording matters; otherwise give an accurate, attributed summary.
At 09:10, Owen reports that two components were moved to stockpile C before Iris arrived. The note can preserve that account and identify a movement record or follow-up as needed. It should not make Iris a witness to the movement. An unsupported account may need verification or different handling under the project's reporting rules; attribution is not a substitute for those requirements.
02.04
Update the record without moving information backward in time
At 10:40, the document coordinator receives fictional movement record MR-008. It records two components moved from staging area B to stockpile C at 08:50 and references DT-448. This is new documentary evidence, received after the visit. Add it through the project's correction or supplement process, distinguishing the reported event time from the time the information entered the record.
A useful supplement explains which question the new record addresses and which questions it leaves open. MR-008 provides an account consistent with eight ticketed and six later visible at B. It does not demonstrate material acceptance, establish the present condition at C, or confirm installation. Retain the earlier discrepancy and link the clarification so another reader can follow how the account developed.
02.05
Give unresolved matters a precise next step
End a substantive entry with a clear status or follow-up when one is needed. Identify the evidence to obtain, the responsible role, and a review point. 'Check materials' is too broad. 'Materials coordinator to link MR-008 to receiving entry RE-019 and refer the lot-document mismatch to the materials reviewer' describes an observable documentation task.
Compare the summary with the underlying notes. Look for stronger verbs introduced during editing: observed becoming verified, reported becoming completed, or received becoming accepted. Open each attachment reference; a clear narrative cannot compensate for a link to the wrong revision or another project's ticket.

Worked example: DR-019 and its supplement
DR-019 entry: '19 October, 09:05, staging area B. Six precast drainage components observed. DT-448, dated 08:35, lists eight components, lot L-72. At 09:10, foreman Owen reported that two were moved to stockpile C before this visit. Movement not observed by Iris. Receiving reconciliation pending.'
Supplement entered at 10:45: 'Document coordinator supplied MR-008 at 10:40. It records movement of two components from B to C at 08:50 and references DT-448. Linked to RE-019 for reconciliation. This supplements the documentary account; no observation at stockpile C or installation record is included.' The lot mismatch with MD-031 remains a separate open issue.

Put the module into practice
A colleague rewrites the case as: 'Eight approved components delivered. Two moved to C. Delivery complete and ready for installation.' They cite DT-448 and MR-008 but omit the visit time, the observed count, and MD-031's conflicting lot. No acceptance or installation decision is in the packet.
Your task
- Identify at least three statements that exceed the supplied evidence.
- Write a corrected entry that preserves both the observed and documentary quantities.
- Write a separate follow-up for the unresolved lot documentation.
Write your response in your own notes before opening the sample. This exercise is for practice and does not record a score or course completion.
Reveal a sample response
'Approved,' 'delivery complete,' and 'ready for installation' are unsupported. The movement statement also needs attribution to MR-008 rather than appearing as Iris's observation. A corrected entry identifies the 09:00–09:25 visit, six observed at B, eight listed on DT-448, Owen's account, and the later receipt of MR-008.
The follow-up can read: 'RE-019: MD-031 identifies L-27 while DT-448 identifies L-72. Materials reviewer to determine the applicable supporting document through the project process. Status: unresolved; preserve both sources.' This maintains a separate record of the discrepancy without deciding which number is wrong.
Keep in mind
Preserve the evidence basis and timing of each statement; reconciliation adds an explanation rather than deleting the original uncertainty.
