Photo records
Field Photo & Evidence Organization Kit
Organize photo references, captions and follow-up records so another reviewer can find the supporting information. Original images stay in your own project storage.
Built for: Civil field staff, construction coordinators and inspectors organizing project evidence.
Use it in your industry
- Index a culvert or roadway observation by station, date and viewing direction.
- Write factual photo captions that separate visible conditions from reported context.
- Hand off photo references and unresolved evidence questions to the next reviewer.
Inside your download
Version 1.2 · 4 guided worksheets · 16 editable text fields, plus instructions, a fictional worked example, and a single-business license.
Photo index entry
Retain original images and metadata under project rules. A filename or caption alone does not establish authenticity.
Prompts include: Project / photo ID / original filename; Capture date and time / recorded timezone; Location and viewing direction; Observed condition / related report or ticket reference.
Caption worksheet
Describe observable facts; do not infer causes, acceptance or hidden conditions from a photograph.
Prompts include: Photo ID / record author; What is directly visible; Context reported by another person / source; Uncertainty or limits of the image.
Evidence handoff
Follow employer and project retention and privacy requirements. Do not place confidential records in a public download.
Prompts include: Project / package reference / sender / recipient; Included file IDs and versions; Transfer method / access check / acknowledgement; Missing records / restrictions / follow-up owner.
Open evidence questions
Close a question only when the response and evidence are recorded. This is an organization aid, not a formal chain-of-custody system.
Prompts include: Question ID / related photo or report; Information still needed / source; Assigned owner / due date; Response received / date / source / closure evidence.
A fictional industry example
Fictional example: Photo P-018 shows standing water beside a culvert access path at station 14+60 at 10:15 a.m., with the recorded timezone and an east-facing view. The caption identifies the project and directly visible condition. It does not claim the water’s source, a hidden defect or acceptance. Question E-03 asks the assigned project representative for the related follow-up record and remains open until a sourced response is received.
How the files work
Download and extract the ZIP. Open START-HERE.html in a current desktop browser with JavaScript enabled. Complete the fields, choose Save a filled copy, and reopen the downloaded HTML to verify your entries. Keep an untouched original.
Edits are not saved automatically. Print / Save as PDF uses your browser's print dialog; review each page before sharing. No installation, account, or internet is needed after download. Mobile file saving varies by browser; desktop use is recommended.
What is not included
No Excel, Word, or pre-made PDF files, spreadsheet formulas, attached-file storage, cloud sync, integrations, automated reminders, AI, or future updates are included. These are documentation and planning aids. They do not provide engineering services, inspection, certification, acceptance decisions, or technical advice.
Project requirements and assigned responsibilities govern. Store your files under your project's rules; the kit does not encrypt them.
